BOARD-APPROVED VOLUNTEER CLEARANCE PROCEDURES

FAIRVIEW SCHOOL DISTRICT
Per Pennsylvania State law, clearances listed below must be submitted before volunteering within a school district. Clearances are valid for 5 years, but must be obtained within 1 year of beginning your volunteer role. Those obtained previously for employment/volunteer purposes are often accepted.
1. PA CRIMINAL HISTORY CLEARANCE (Act 34) (FREE OF CHARGE)

2. CHILD ABUSE HISTORY CLEARANCE (Act 151): (FREE OF CHARGE)

3. FBI FEDERAL CRIMINAL HISTORY – FINGERPRINT (Act 114):
($21.35 for FBI Fingerprint)
- Register online at https://uenroll.identogo.com. Enter 1KG6Y3 then provide fingerprint and payment at selected location.

4. CHILD ABUSE REPORTING TRAINING (Act 126): (FREE OF CHARGE)
-This is a 4 hour online training that can be done at your own pace in the comfort of your own home. Please call the Central Administration Office Secretary (814‐474‐3170) or email volunteer@fairviewschools.org to schedule this training.
5. TUBERCULOSIS (TB) TEST:
- Results of a TB skin test from a licensed healthcare provider will need to be submitted prior to board approval.
Please be advised it can take up to 2-3 weeks to receive results for all clearances.
