BACKGROUND

Starting in January 2018, Fairview Middle School awards the FMS Citizenship Award to one student each month, while school is in session, whose lives have been shaped by service and whose service-centered actions are making "a ripple effect" in our community. 

PURPOSE

The FMS Citizenship Award is designed to recognize outstanding students who significantly contribute to the community.

CRITERIA FOR NOMINATING AN FMS CITIZENSHIP AWARD RECIPENT

Students may be nominated for this award for their exemplary achievements or efforts in the following areas:

•           Significant contribution to the community

•           An inspirational role model for the school community

•           The scope of impact the individual’s contribution has in the local area

•           Accepting responsibilities with the community by helping others in need

•           Respecting and adhering to rules for socially responsible & desirable behavior

•           Promoting harmony & inclusiveness

•           Interacting positively with adults & other children

•           Resolving issues in peaceful & non-threatening ways

•           Willingness to participate actively & make a positive contribution to others

HOW TO NOMINATE

Complete the nomination form.  Forms can also be found on Schoology or in the FMS office.  Completed forms should be submitted to the FMS office or emailed to the principal or school counselor.  Nominations will be reviewed by a committee and the recipient will be announced monthly.
