Access our new online registration portal here and click "Create Account". Complete and submit online enrollment.

Procedure for 2017/18 Enrollment/Option 2 for 2018/19 Enrollment
The forms below can be filled out electronically and then printed or printed and then filled out manually. All forms will need to be physically signed. They
 are also available in the offices at Fairview High School, Fairview Middle School, and Fairview Elementary School.

Step 2 - Schedule an Appointment
Please contact the Central Registrar, Jonathon Valentin, at 814-474-3285 or at enrollment@fairviewschools.org to schedule an appointment, to ask any questions you have, or to get help with the Online Registration Portal.

Step 3 - Meet with the Registrar
Location:
Fairview High School - 7460 McCray Rd, Fairview, PA 16415
Park in the South Lot (near the tennis courts) and enter through the doors on the left
Registrar's Office for all students K-12 is located in the High School

Please bring the following items with you when you enroll your student:
- Registration Forms (not required if submitted online)
- Proof of Age (Student's Birth Certificate or Passport)
- Two Proofs of Residency
Primary Proof of Residency (property deed, current lease, current mortgage statement, current property tax bill, or current utility bill—electricity, gas, garbage, or street light)
Secondary Proof of Residency (Second document from above list, current bank statement, current cell phone bill, current credit card bill, current pay stub, valid driver’s license, valid state identification card, or valid vehicle registration card)
- Immunization Records
- Parent Photo Identification (Driver's License, Military ID, Passport, or State ID)
- Custody Agreements/Court Orders (if applicable)
- Record of physical exam for students beginning grades K, 6, or 11 (if available)
- Record of dental exam for students beginning grades 1, 3, or 7 (if available)

Kindergarten Age Requirement: Incoming kindergarten students for the 2018-2019 school year must have a date of birth on or before August 31, 2013.
