LIMITED CONTACT VOLUNTEER INSTRUCTIONS

FAIRVIEW SCHOOL DISTRICT

A LIMITED CONTACT VOLUNTEER is defined as a volunteer who is always within proximity of a district professional employee and there is no time in which the volunteer has the potential to be alone with a student or students. Volunteers considered limited contact include, but are not limited to, classroom helpers, party helpers, party coordinators, library/office helpers, classroom field trip chaperones and PTO/PTA.

Per Pennsylvania State Law, clearances must be submitted before volunteering within a school district. Limited Contact Volunteer clearances are valid for five (5) years from the date on the clearance and may be submitted at any point during those five years. Clearances obtained previously for other volunteer/employment purposes will be accepted.


 Please be advised it can take up to 2-3 weeks to receive results for all clearances.

1. COMPLETE THE FOLLOWING CLEARANCES:    
a. PA Criminal History Clearance (ACT 34) (FREE)

b. Child Abuse History Clearance (ACT 151): (FREE)

c. FSD Affidavit OR FBI Fingerprint Clearance (ACT 114):
(FREE for Affidavit / $23.25 for FBI Fingerprint)

* Have you been a resident of Pennsylvania for ten (10) consecutive years or more?

  • If YES: Complete the FSD Affidavit for Volunteer Service.  This document must be notarized by a Notary Public certified in the State of Pennsylvania.  A notary is available in the Fairview Middle School office from 7:30am-3:00pm M-F, and the Fairview Elementary School office from 8:00am-3:30pm M-F.  Please call 814-474-3424 (FMS) or 814-474-3122 (FES) if you would like to schedule an appointment. Document must be signed in the presence of the Notary.

2. COMPLETE THE ONLINE VOLUNTEER APPLICATION

a. Go to https://apps.raptortech.com/Apply/NDU2NTplbi1VUw==

b. Fill out the application and upload your clearances.

c. Sign the Volunteer Disclaimer and submit the application.

d. Email notification will be sent when the application is approved.
